Relapse Prevention Plan

relapse prevention planIn some ways, returning home from rehab is one of the most challenging parts of the recovery process; it is during this stage that relapse becomes a major concern. That is why Rise Above provides clients with a relapse prevention program, allowing clients to gradually transition back into sober living while receiving ongoing support and encouragement from sponsors, peers, and addiction therapists. If you are interested in learning more about relapse prevention or any of our Murrieta rehab programs, contact Rise Above by calling (877) 641-0717.

Prevent Post-Rehab Relapse

Relapse prevention starts during residential rehab. At Rise Above, our clinicians will work to create a detailed discharge plan with you as your inpatient rehab experience draws to a close. This plan provides our clients with the resources they need to transition successfully out of Rise Above’s residential drug treatment facility and into their everyday sober life. Relapse prevention resources may include outpatient recovery services, partial hospitalization, or a less structured version of our residential program for some clients. Our outpatient rehab clients will meet with their individual therapists to assess what they need in order to continue on the path of gratitude and recovery.

Maintaining Addiction Recovery

The recovery journey does not end when a person finishes an inpatient addiction recovery program. Recovery from alcohol and drug addiction is a process that lasts a lifetime. Returning to everyday sober living can present completely new and familiar challenges, emotions, and stressors. When not properly addressed or managed in healthy ways, these stressors are likely to trigger a relapse. Relapse prevention starts in the following programs:
Though relapse itself does not imply that their recovery has failed, relapse is certainly something to be avoided whenever possible. To ensure that our Rise Above clients are fully equipped, we work with them to establish a solid foundation in recovery and build community connections. Having the tools, strategies, and phone numbers in place before a drug treatment program is completed is a safeguard against relapse.

Relapse Prevention Plan

The hallmark of our relapse prevention system is the development of a relapse prevention plan. This is a detailed discharge document that clients will help develop. Sponsors and therapists will contribute to the relapse prevention plan. This living document will provide ongoing strategies, phone numbers, and resources to help people transition from inpatient rehab to sober living outside of addiction treatment. In some ways, returning home from inpatient drug rehab is one of the most challenging parts of the drug rehab process. It is during this post-rehab stage that relapse becomes a significant concern.

Alcohol Relapse Prevention

For people whose concern is alcohol relapse, our alcohol relapse prevention plans will provide some options to help with mitigating daily struggles, stressors, triggers, and temptations. Being prepared helps ensure a smooth return to everyday sober living.
